The distal upper extremity is a different clinical problem than the shoulder. Thinner tissue, smaller joints, and a musculotendinous architecture under constant repetitive load. ZHOU works the yang channels of the elbow, wrist, and hand, where tendinous adhesion and channel obstruction with numbness and tingling are the characteristic patterns. The formula is lighter in its blood-moving action and heavier in its sinew-relaxing and wind-damp-clearing work than JIAN.

San Qi leads at 15%, moving blood and resolving stasis in the poorly perfused sinew of the distal arm. Wei Ling Xian at 13% is the formula's signature herb, dispelling wind-damp and softening the fixed, adhesive joint stiffness that defines chronic elbow and wrist presentations: the lateral epicondyle that will not loosen, the locked finger tendon, the wrist channel thick with stagnation. Qiang Huo and Du Huo open the Du Mai into the arm. Bai Zhi conducts to the radial forearm and wrist, Fang Feng to the lateral elbow, and Gui Zhi keeps the whole arm channel open from shoulder to fingertip. Ru Xiang leads the resin cluster at the sinew layer, Yan Hu Suo carries the analgesic weight these pain-dominant presentations need, and Bing Pian opens the surface to drive the formula in.

Indicated for: Subacute and chronic elbow, wrist, and hand presentations from Day 3 onward. Lateral and medial epicondyle tendinopathy that has not resolved, wrist channel obstruction with palm and finger numbness or tingling, finger tendon adhesion and locking, radial wrist tendinopathy, and chronic small-joint stiffness of the hand from channel obstruction. Begin at Day 3, transitioning from CHONG.

Application: 3 to 5 sprays to the affected area. Massage in firmly for 2 to 3 minutes until absorbed. Two to three times daily. For chronic epicondyle presentations, apply and wrap the elbow with a warm cloth for 20 to 30 minutes, since the poorly vascularized tendon attachments respond to sustained warmth. For finger tendon adhesion, work the full length of the tendon, not just the point of locking.
